But more recently, scientists have started asking a different question. What if this relationship also works the other way around? What if poor skin health doesn't just reflect what's happening inside the body, but actively influences it?
The evidence suggests it does. Your skin is not a passive surface. It's the largest organ in the body and is in constant communication with your immune system, your circulation, and your cells. That means what happens in your skin doesn't necessarily stay in your skin.

Why Does Skin Inflammation Affect the Whole Body?
Your skin is under constant daily stress from UV radiation, pollution, and temperature changes, all of which cause damage to skin cells. In younger skin, where NAD+ levels are higher and cellular repair is working efficiently, that damage is repaired quickly. But as we age, NAD+ levels decline, repair becomes less effective, and damage starts to accumulate.
Some of those damaged cells reach a point where they can no longer repair or renew themselves. Rather than dying off, they remain in the tissue in a state called cellular senescence. Studies of human skin biopsies have found that the proportion of senescent cells in the epidermis rises from around 20% in younger skin to as high as 60% in older skin, a striking accumulation driven largely by decades of UV exposure and environmental stress.
This is a cause for concern because these senescent cells don’t perform their normal functions, such as producing collagen or elastin. Instead, they secrete a stream of inflammatory signals known as the SASP (senescence-associated secretory phenotype), which enter the circulation and travel throughout the body, contributing to what is known as inflammaging: the chronic, low-level inflammation that is a key driver of biological aging. As the skin is the largest organ in the body, even a modest accumulation of senescent cells and the inflammation they generate represents a significant biological burden on the whole body.
